Confidential Hazardous Incident Reporting Programme

 

What is CHIRP?

CHIRP provides an independent confidential, although not anonymous, reporting system which enabled individuals to submit reports regarding hazardous incidents. With respect to the maritime sector the primary aim of CHIRP is to enhance the safety both in the UK and worldwide.
It’s Maritime Programme was founded in 2003 and was originally funded by the UK Government. CHIRP it is now funded by sponsorships from The Corporation of Trinity House, The Lloyds Register Foundation, Britannia P& I Club, International Foundation to Aids to Navigation and Cammell Laird.

Confidential incident reports are submitted by individuals both in the UK and worldwide. Upon receipt of a report CHIRP staff validate and review it. The ultimate objective of CHIRP is to make the report as widely available as possible whilst always protecting confidentiality of the reports source. CHIRP do not normally act upon completely anonymous reports as they cannot be validated. When appropriate the report information is discussed with relevant agencies with the aim of finding a resolution. CHIRP only shares disidentified data with third party organisations and the confidentiality of the source is assured in any contact with an external organisation.

CHIRP never retains personal details from reports received. After checking that the report contains all relevant information, all personal details are returned to the reporter with an acknowledgement letter. Each report is allocated a unique reference identification. After the return of personal details, CHIRP is unable subsequently to contact the reporter. The reporter may, if he/she wishes, contact the CHIRP office for additional information by using the report reference identification.

A quarterly newsletter entitled FEEDBACK is published covering current topics from disidentified reports and the complete archive of reports can be accessed below. Some reports are disidentified further (e.g. time and place, company, reporter’s gender) before being published in FEEDBACK, or making any data available to other agencies. FEEDBACK maintains an awareness of Human Factors issues within the sector and provides a forum for discussion.

 

How can I report an incident?
